29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about 06/18/2014, at the jobsite at 2509 Cypress Drive, St Charles, MO,  a 16' aluminum ladder was used as a means of egress in a 25' long excavation that measured approximately 9' 6" up to a maximum of 10' 8" deep.  The ladder side rail extended approximately 1' 6" above the upper landing surface instead of the 3' that is required by the standard.
