29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about June 19, 2014, at 1737 State Hwy 46, New Braunfels TX, the employer failed to ensure an employee was protected from a fall hazard. Employee was observed working from an aerial basket without fall protection. Employee was installing a piece of metal track. Employee exposed to a fall hazard of approximately 8 feet.
