29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) Seven Hills Subdivision, Lot #51, Dallas, GA; The employer allowed employees to wear body harnesses with the attachment points that were located near the waist while installing roof felt and shingles on a 9:12 pitched roof, exposing the employees to fall hazards of approximately 18 feet. The Marcelo Cardenas-Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 898041, citation number 01, item number 02 and was affirmed as a final order on 04/25/13, with respect to a workplace located at 1655 Cavan Drive SW, Marietta, GA 30064.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Seven Hills Subdivision, Lot #51, Dallas, GA; The employer exposed employees to fall hazards of approximately 9 feet as employees were allowed to access a ladder with side rails that were raised approximately 2 feet above the landing. The Marcelo Cardenas-Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 898041, citation number 01, item number 03 and was affirmed as a final order on 04/25/13, with respect to a workplace located at 1655 Cavan Drive SW, Marietta, GA 30064.
