29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a) Profit St., Room 6A/B, back-left side: On or about June 19, 2014, employee(s) were exposed to fire and chemical release hazards from a blocked exit door, which was obstructed with approximately six large product boxes, as well as wooden pallets, inhibiting evacuation of employees in the event of an emergency.  b) AT&T Facility, Dock C: On or about June 20, 2014, employee(s) were exposed to fire and chemical release hazards from a blocked exit door, which was obstructed with a wooden pallet, stored welding equipment, and was locked with a metal bar, inhibiting evacuation of employees in the event of an emergency.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a) AT&T Facility, Dock A: On or about June 20, 2014, employee(s) were exposed to electrical hazards from an electrical junction box which was not fully closed and susceptible to condensation accumulation in a refrigerated facility.  b) AT&T Facility, Room #7: On or about June 20, 2014, employee(s) were exposed to electrical hazards from an electrical junction box on the right, upper wall of the refrigerated room, which was not fully closed and susceptible to condensation and ice accumulation.  c) AT&T Facility, Dock B, Upper Wall: On or about June 20, 2014, employee(s) were exposed to electrical hazards from an electrical panel (Model: QO6-12L 100S) on the back, upper wall, which was not fully closed and was accessed for maintenance tasks.  d) AT&T Facility, Dock B, Near Office: On or about June 20, 2014, employee(s) who perform maintenance tasks were exposed to electrical hazards from an electrical panel box (Panel HVP1) which was missing a blank panel and was not fully closed.
