29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about June 19, 2014 and times prior thereto, an employee of Advance Sign and Lighting, Inc. was observed working in an aerial lift for the installation of a decal on the Citgo store located at or near 1715 E. Johnson Street in Jonesboro, Arkansas.  Fall protection was not used when working inside of the basket, exposing the employee to fall hazards.
