29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about June 19th, 2014, an employee was operating a cement mixer using an extension cord which had the insulation pulled back around the plug area exposing the conductors and causing pull to joints or terminal screws.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about June 19th, 2014, an employee was engaged in masonry work on the walls of a residential structure while working on a thirteen (13) foot scaffold without ladder access, and the cross braces were used for access.
