29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Location: 223-227 Union Street, Lynn, MA: On or about June 18, 2018, employees working on the second floor of a multi-story building having unguarded sides/edges were exposed to a fall of 12+ feet to the concrete floor or hard ground below.  b) Location: 223-227 Union Street, Lynn, MA: On or about June 18, 2014, employees were exposed to fall hazards from the first floor to the basement because of improper or missing guardrails.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Location - 223-227 Union Street, Lynn, MA: On or about June 18, 2014, employees were not adequately or effectively trained to recognized fall hazards and take the appropriate actions to minimize fall hazards. Employees were exposed to unguarded sides of floors and floor opening/holes.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a) Location - 223-227 Union Street, Lynn, MA: On or about June 18, 2014 the employer did not provide employees using ladders or stairways effective or adequate training to ensure proper use.  Extensions ladders used to gain access to the second floor did not extend at least three (3) feet above the dismount point.
