Employees are exposed to petroleum coke in the workplace. The employees have not been trained on the hazard communication standard or the hazards of petroleum coke, which include respiratory and combustible dust hazards.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and -2 on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced in the work place: (a) On or about June 23, 2014, it was learned that the employer did not have a training program in place for the hazard communication program and the chemicals to which employees are exposed. Employees are exposed to petroleum coke in the workplace. The employees have not been trained on the hazard communication standard or the hazards of petroleum coke, which include respiratory and combustible dust hazards.

29 CFR 1917.26(d)(2): Stretchers were not kept in close vicinity of the vessel and/or positioned so as to avoid damage to the stretcher: (a) On or about June 23, 2014, it was learned that the employer was storing the stretcher in the garage attached to the office, which was located at the far end of the facility across railroad tracks. The employees were not aware that the stretcher was at the facility.
