29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about June 19, 2014, at the above addressed job site, employees were exposed to the hazard of falling, while doing roofing work on a commercial building while working from a height over 14 feet to the ground below without proper fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.  On or about June 19, 2014, at the above addressed job site, an employee was exposed to the hazard of falling, while using a portable extension ladder which was not extended 3 ft. above the landing surface.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:   a) On or about June 19, 2014, at the above addressed job site, employees were exposed to the hazard of falling while  using a portable extension ladder which was not secured from displacement.
