29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about June 20, 2014, at 2101 FM 1101, New Braunfels, TX, the employer failed to ensure the employee who was helping to cut a cement driveway had proper eye protection. Employee was not wearing any eye protection and was adding water to keep the cement blade cool. Employee was exposed to an eye injury from being struck by cement particles.
