29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide. (a) Consult Room - On or about June 17, 2014 employees were exposed to a fall hazard while working on a scaffold with a walkway less than 18 inches wide. The scaffold walkway consisted of one 10 inch board.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Rafters - On or about June 17, 2014 three employees were observed walking and working on the frame of the building without guardrail system or personal fall arrest system. The employees were 11.2 feet above concrete.
