29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:     On or about June, 19 2014, at the above addressed jobsite, employees patching a vault in a trench measured at approximately 6.7-7 feet.  The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
