29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   (a)  Front Scaffolds - On or about June 19, 2014 employees were exposed to head injuries while working beneath the scaffold with no hardhats. Employees were handling bricks and mortar above the ground level employees.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  (a)  Rear Scaffold - On or about June 19, 2014 an employee was exposed permanent eye injuries while cutting bricks with no eye protection.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross-braces shall not be used as a means of access.  (a)  Rear Scaffold - On or about June 19, 2014 an employee was exposed to a fall hazard while working on a scaffold that had no ladder installed. One employee climbed the frame 14 feet above the ground to access the scaffold.     (b)  Front Scaffolds - On or about June 19, 2014 five employees were exposed to a fall hazard while working on scaffolds that had no ladders installed.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.   (a)  Rear Scaffold - On or about June 19, 2014 an employee was exposed to a fall hazard while working on a scaffold 14 high with no guardrail system or personal fall arrest system.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.   (a)  Rear Scaffold - On or about June 19, 2014 an employee was exposed to a fall hazard while working on a scaffold that was missing cross-braces on the front and the rear sides.
