29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier, signal word, hazard statement(s), pictogram(s), precautionary statement(s) and name, address, and telephone number of the chemical manufacturer, importer, or other responsible party: (a) On or about June 17, 2014 the inspection revealed containers of WYO-VIS polymer viscosifier with deteriorated labels that no longer indicated the contents. The containers were located on the pump truck at 201 SE Swan Ave in Siletz, Oregon.
