29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about June 20, 2014 and times prior thereto, employees of Joe Rivera were observed roofing a steep sloped rooftop of a one story residence being constructed at or near 1562 McAfee Circle in Cabot, Arkansas. The employees using nail guns were not provided with eye protection, exposing the employees to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 20, 2014 and times prior thereto, employees of Joe Rivera were observed roofing a steep sloped rooftop on a residence being constructed at or near 1562 McAfee Circle in Cabot, Arkansas. Employees observed worked on a steep slope rooftop with a 8:12 pitch and no fall protection, exposing the employees to falls of distances greater than 6 feet to the ground level below.
