29 CFR 1910.147(c)(4)(ii)(B): The employer had not developed Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:      (a)  On June 20, 2014, in the main part of the shop, employees were exposed to serious injuries, in that the ABC Index Machine had been taken down for maintenance due to a blown hydraulic line without the benefit of developed and written Machine Specific Lock Out procedures for shutting down and and controlling the hazardous energy sources to the equipment.             Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    (a)  On June 20, 2014, in the Milling Area of the facility, employees were exposed to serious injuries from rotating parts in that the Balcor Pedestal Grinder was in use without a functioning and properly adjusted work rest.         Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch:      (a)  On June 20, 2014, in the Milling Area of the facility, employees were exposed to serious injuries from rotating parts in that the wheel of the Balcor Pedestal Grinder had a decreased diameter that could not be adjusted to within one-fourth of an inch distance between the wheel and the wheel peripheral member (hood guard) and or an adjustable tongue guard.         Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination shall be protected from accidental contact or breakage by a suitable fixture or lamp-holder with a guard:              (a)  On June 20, 2014, in the Shop Supervisor's Office, employees were exposed to serious injuries in that the florescent ceiling light fixture was not protected from accidental contact or breakage. The florescent light bulb was 76-1/2 inches from the floor without a guard or protective sleeve to prevent potential broken glass shards from causing injuries if contact was made.               Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
