29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about May 29, 2014, a roofing crew was using a pneumatic nail gun while placing shingles on a residential roof at 1118 Bromfield Terrace, Manchester, MO without any means of eye protection, exposing them to a potential eye injury. Angelo Martinez was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 315596403, citation number 1, item number 1 and was affirmed as a final order on November 11, 2011, with respect to a workplace located at 3614 Harmann Estates, Bridgton, MO 63044.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: On or about May 29, 2014, four employees were exposed to a fall from an elevation at 1118 Bromfield Terrace, Manchester, MO when they were working on a residential roof without any means of fall protection. A fall protection plan, as outlined in 1926.502(k), was not present at the worksite, nor was there a safety monitoring system in place at the time of inspection for low-sloped roof work. Angelo Martinez was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 315596403, citation number 1, item number 4 and was affirmed as a final order on November 11, 2011, with respect to a workplace located at 3614 Harmann Estates, Bridgton, MO 63044.
