29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: (a) 500 Gallher Drive, Fairmont WV: An employee used a Dewalt Circular Saw, s/n 851678, to cut wood. The electrical plug for the saw plugged into an extension cord which was daisy chained with two other extension cords, as determined on or about 06/18/2014. ABATEMENT CERTIFICATION REQUIRED
