29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Signode Strapper Area - On or about June 7, 2014, an employee did not follow the specific lockout/tagout procedures exposing employees to struck-by and caught-between hazards when performing maintenance work on the 'Mainline Strapper 1st Platen' (Signode Compression Strapping Machine, Model: CLM8TTT, Serial Number: 1725). An employee did not remove the key and keep the key in his/her possession until servicing/maintenance had been completed.
