29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:  (a) Roof - Employees worked installing a new roof on the 40-foot by 60-foot building.  The flat roof had a height of 9 feet above the ground on the south side and a height of 13 feet above the ground on the north, west, and east sides.  The employees did not use a safety monitoring system or any other fall protection system to prevent them from falling to the ground below, on or about June 20, 2014.  No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Roof - Employees worked installing a new roof on the building.  The employees used an extension ladder to access the roof, and this ladder extended to about one foot below the landing level of the roof, exposing employees to a fall hazard, on or about June 20, 2014.  No abatement certification or documentation required.
