29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Facility, Fabrication Shop: On or before June 23, 2014, employees were exposed to fall hazards of approximately 17 feet 7 inches while installing floor grating on a fabricated structure. The employer did not assess the workplace to determine what personal protective equipment (PPE) was necessary to allow horizontal travel while exposed to fall hazards.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: (a) Facility, Fabrication Shop: On or before June 23, 2014, employees were exposed to tripping hazards in that flat bar stock was in use between the welding tables. b) Facility, Fabrication Shop: On or before June 23, 2014, employees were exposed to slipping hazards in that equipment leaking hydraulic fluid was not repaired to eliminate the hazard.
