The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: a) On or about 6-23-14, employees were working from a low slope roof surface with a roof edge ten (10) feet above concrete surface without utilizing fall protection equipment.

29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces: a) On or about 6-23-14, employees who were accessing/exiting a roof surface were using a 24 ft. extension ladder positioned on a grassy surface without utilizing the slip resistant feet to secure the ladder. b) On or about 6-23-14, employees were working from a 20 ft. extension ladder positioned on a grassy surface without utilizing the slip resistant feet to secure the ladder.
