29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: a) J & L Building Materials, Inc. - The employer placed wooden pallets of asphalt roofing shingles supported on approximately a four inch steel lip at a height between 4 and 15 feet above ground level. The physical condition of the wooden pallets [e.g. dry rotted and bowing] had destabilized and the weight of the supplies stored upon it has increased the possibility of the materials and pallets collapsing on employees and/or to the ground surface. This was observed on June 26, 2014.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent. a) J & L Building Materials - The employer stored heavy equipment and supplies on shelving in warehouse one - door one, which did not have an approved load rating attached to any of its structures. This was observed on June 23, 2014.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: a) J & L Building Materials - The employer did not ensure that an adequate inspection of the propane powered Toyota industrial trucks was conducted prior to being used at the facility. A propane powered industrial truck being used in the warehouse had a horn that was not operational. This was observed on June 23, 2014.
