29 CFR 1910.37(a)(3): Exit routes were not free and/or unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp were not provided where the exit route was not substantially level: a) On June 23, 2014, The exit route was obstructed. There were metal racking beams obstructing the passageway to the exit route in the new area of the Plastics Department. b) There was another exit route that was obstructed in Department 59, between Department 57 and the Plastics Department. There was sheet metal in baskets obstructing the exit.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made. Aisle(s) and passageway(s) were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard: a) On or before June 23, 2014, located in the Plastics Department, at the P30, Q28, and P25 machines, the employees were exposed to potential struck by hazards in that the employees' work area encroached the aisle ways shared with forklift traffic exposing workers to struck by hazards.

29 CFR 1910.178(m)(1): Trucks were driven up to employees standing in front of benches or other fixed objects: a) Prior to June 23, 2014, an employee, who was working on the P07 machine in the Plastics Department, was putting scrap into log, the forklift driver did not sound their horn and employee almost got hit by a forklift driver. b) On July 15, 2014, Fork lift driver drove to employee's work area, employee was behind fork lift working, employee extended hand to not allow forklift to back up closer to employees' space; however, fork lift started backing up and several employees yelled to fork lift driver to not run over employee.
