29 CFR 1910.27(c)(1): On fixed ladders, the perpendicular distance from the centerline of the rungs to the nearest permanent object on the climbing side of the ladder was not 36 inches for a pitch of 76 degrees, and 30 inches for a pitch of 90 degrees with minimum clearances for intermediates pitches varying between these limits in porportion to the slope, except as provided in subparagraphs (3) and (5) of this paragraph: T-Point: On, or about, June 24, 2014, the fixed ladder that provided access to the overhead baggage conveyor and catwalk, located at the baggage claim conveyor labeled as Bag Belt 1, was not provided with adequate clearance on the climbing side. The catwalk restricted access at the top of the ladder and the baggage claim conveyor frame blocked access to the lower rungs at the bottom of the ladder.

29 CFR 1910.27(d)(3): The side rails of through or side-step ladder extensions did not extend 3 1/2 feet above parapets and landings. T-Point: On, or about, June 24, 2014, the ladder extensions provided on the fixed ladder located near the main conveyor control console were not long enough and adequate headroom was not provided to employees who accessed the catwalk from the ladder. 29 CFR 1910.27(c)(2): A clear width of at least 15 inches (0.4 m) was not provided each way from the centerline of ladder(s) in climbing space(s): T-Point: On, or about, June 24, 2014, the following fixed ladders that provided access to the overhead baggage conveyor and catwalk were not provided with adequate side clearance: A. Ladder at Main Conveyor Control Console: The catwalk landing was located too close to the right side rail on the ladder. B. Ladder at Baggage Claim Conveyor Labeled as Bag Belt 1: The upper section of the ladder was not provided with adequate side clearance due to the close proximity to the catwalk landing on the left side and conveyor frame on the right side. Also, an electrical box labeled as Bag Belt #1 control panel, MCP/CD4, was located in close proximity to the left side of the ladder at the floor level.

The Bagbelt System was not turned off using the control buttons provided (e.g., stop control, emergency stop control), instead, the employees relied on the sensor trip function to turn off the affected portions of the conveyor system.

29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not comply with 29 CFR 1910.147(c)(3): T-Point: On, or about, June 24, 2014, the Bagbelt System Tagout Procedure contained in the company's Lockout/Tagout program did not afford Full Employee Protection to employees who cleared conveyor belt jams. The conveyor system was capable of being locked out at numerous control stations located along the conveyor system, including at the main control console and along the catwalk. Locks were provided for use that were affixed to each control station by a chain. The Bagbelt System Tagout Procedure did not include any additional measures to secure the conveyor system to level equivalent to affixing a lock to one of the control station devices provided.
