Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about June 24, 2014, at the above addressed site, employees were exposed to a fall hazard when performing roof work on a low-slope roof with unprotected edges approximately 10 feet 6 inches in height while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: On or about June 24, 2014, at the above addressed site, employees were exposed to a fall hazard when gaining access to and from a roof using a portable ladder that was not secured to prevent accidental displacement.
