29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about June 17, 2014, at the jobsite located at 10915 Eicher Dr., Lenexa, KS: An employee engaged in electrical installation activities, using a job-made man basket and forklift, was exposed to fall and struck-by hazards in that comprehensive scaffold training to include pre-use examination and hazard identification had not been provided.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: On or about June 17, 2014, at the jobsite located at 10915 Eicher Dr., Lenexa, KS: An employee engaged in electrical installation activities, to include light fixture installation, from a job-made man basket was exposed to fall hazards of approximately 20 feet from the elevated forks to the ground below in that the man basket was not secured to the forklift mast.
