29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:    a) On or about June 24, 2014 - at the jobsite located at 6728 S. Western Avenue, Chicago, IL. - employees performing piping operations in a trench measuring approximately nine (9) feet eight (8) inches in depth, five (5) feet in width, and nine (9) feet in length were using inadequate shoring in the trench, thereby exposing the employees to cave in hazards.

29 CFR 1926.652(g)(2): Additional requirement for shield systems used in trench excavations. Excavations of earth material to a level not greater than 2 feet (.61 m) below the bottom of a shield shall be permitted, but only if the shield is designed to resist the forces calculated for the full depth of the trench, and there are no indications while the trench is open of a possible loss of soil from behind or below the bottom of the shield:  a) On or about June 24, 2014 - at the jobsite located at 6728 S. Western Avenue, Chicago, IL. - employees performing piping operations in a trench measuring approximately nine (9) feet eight (8) inches in depth, five (5) feet in width, and nine (9) feet in length were using a trench box which had approximately two (2) and half (1/2) feet of unprotected soil under the (shoring) shield system, thereby exposing the employees to cave in hazards.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support   that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.  In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:    a) On or about June 24, 2014 - at the jobsite located at 6728 S. Western Avenue, Chicago, IL. - employees performing piping operations in a trench measuring approximately nine (9) feet eight (8) inches in depth, five (5) feet in width, and nine (9) feet in length were using an extendable ladder that was not extended three (3) feet above the upper landing surface of the trench, thereby exposing the employees to fall hazards.
