29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 21 Woodworth Street, on or about 3/20/2014: Employees were Installing residential roof shingles 6 feet or more above the lower working level without the use of any personal fall arrest system.   b) 19 Woodworth Street, on or about 3/20/2014: Employees were Installing residential roof shingles 6 feet or more above the lower working level without the use of any personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a) 21 Woodworth Street, on or about 3/20/2014: Employees were using two ladders to reach the upper work surface that were not extended a minimum of 3 feet above the upper landing surfaces. Ladders were extended approximately 1 feet 6 inches and 2 feet 6 inches above each surface.
