1904.39(a): The employer failed to report to OSHA a work-related death within eight (8) hours after the death of the employee. Work site: On or about June 24, 2014, in the junk yard located at 1321 Darrington Rd., an employee sustained multiple blunt trauma injuries by a forklift causing death. The employer failed to notify OSHA of an employee death in a timely manner. The employee was exposed to a fatal struck-by hazard.
