29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:     (a) Jobsite, Cunningham & Main Streets, Butler, PA:  On or about 06/24/14 an employee was welding from inside an aerial lift without wearing a harness and lanyard attached to the boom or basket exposing the employee to fall hazards of approximately 18-20 feet to the ground.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:     (a) Jobsite, Cunningham & Main Streets, Butler, PA:  On or about 06/24/14 two employees engaged in steel erection activities were not protected from falling by the use of guardrails, safety nets, personal fall arrest, positioning devices or fall restraints.  The employees were working at the edge approximately 20 feet above the ground laying roof deck panels and were not wearing any personal fall arrest equipment nor were any other fall protection systems in place.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:     (a) Jobsite, Cunningham & Main Streets, Butler, PA:  On or about 06/24/14 retraining was not provided to an employee who was working in an aerial lift without wearing a harness and lanyard attached to the boom or basket indicating that the employee failed to retain the requisite proficiency and skill to work safely.
