29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Near the well head:  On or about June 19, 2014, employees engaged in the installation of the well's flow line used a forklift to lift and position a valve assembly.  The forklift operator had not been trained.  This exposed the employees to the hazard of being struck by a falling load.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:  Near the well head:  On or about June 19, 2014, employees engaged in the installation of the well's flow line used a forklift to lift and position a valve assembly.  The valve assembly was suspended by a chain sling looped around one of the forklift tines.  This exposed the employees to the hazard of being struck by a falling load.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)  South side of the well site:  On or about June 19, 2014, and times prior to, a new employee applying a mastic to piping during the installation of the well's flow line had not been trained on the hazards of the mastic.  This exposed the employee to the hazards of the mastic as its health effects on the body and fire.

The two employees conducting the coating process were not wearing protective eyewear exposing them to the hazard the mastic contacting their eyes.
