29 CFR 1910.23(a)(4):Skylights were not guarded by a standard skylight screen or a fixed standard railing on all exposed sides:  Building Rooftop, Above Staging Area: On or about June 24, 2014, an employee performing maintenance and repair to rooftop skylights, which lacked fixed guardrails on exposed sides and/or skylight screens, was exposed to fall hazards of approximately 20 feet to the warehouse concrete ground below.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  Manufacturing, Slurry Machine: On or about June 30, 2014, and times prior there to, employee(s) who operate the "Slurry" Machine for candle manufacturing processes were exposed to continuous noise at a dose of 97.8% (8-Hour Time Weighted Average sound level of 85 dBA) during the 444 minute sampling period. For a full eight hour work shift this exposure is equivalent to an average sound level of 89.8 dBA. Exposure calculations include a zero increment for the 36 minutes not sampled.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten liquids, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Decorating Department: On or about June 24, 2014, and times prior there to, employee(s) who use, handle, and transport chemicals, such as but not limited to, Gemini Coating Clear Gloss Lacquer (Toluene 45.84%, Xylene 6.77%, Ethyl Benzene 1.70%), CT-66/CT-1205 Color Thinner (25-35% Xylene, 20-30% Methyl Ethyl Ketone), and various Pylam Products Company Dyes, including Pylam DarK Blue LX-11452, Pylam Golden Yellow LX-10921, Pylam New Red LX-11371, were exposed to eye injury hazards and were not provided with or required to use adequate eye or face protection by the employer.  b) Manufacturing Department and Decorating Department: On June 24, 2014, and times prior there to, employees who perform manufacturing and decorating processes where glass candle jars are handled were exposed to eye injury hazards from broken glass fragments and were not provided with adequate eye protection, such as but not limited to safety glasses.
