29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:    1) Jobsite - On or about June 25, 2014 at 2 Vitucil Court, Cohoes, NY 12047 an employee was working as the ground level person cutting siding and providing cut siding to installers at an elevated location without wearing safety glasses or a hard hat.  Accordingly, he was exposed to a struck by hazard to the eyes.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    a)  Jobsite - On or about June 25, 2014 at 2 Vitucil Court, Cohoes, NY 12047 a frayed extension cord was being used and connected to a Mikita skill saw that was being utilized as part of a siding installation job exposing employees to an electric shock hazard.
