29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    The employer does not guard the point of operation on machines whose operation exposes employees to injury.       a. This violation occurred on or about June 24, 2014, in the fabrication shop where employees were exposed to struck-by and caught-by hazards while operating a Carlton brand radial drill identified as Drill #1 without a point of operation guard.       b. This violation occurred on or about June 24, 2014, in the fabrication shop where employees were exposed to struck-by and caught-by hazards while operating an unknown brand radial drill identified as Drill #2 without a point of operation guard.       c. This violation occurred on or about June 24, 2014, in the fabrication shop where employees were exposed to struck-by and caught-by hazards while operating an unknown brand radial drill identified as Drill #3 without a point of operation guard.            d. This violation occurred on or about June 24, 2014, in the fabrication shop where employees were exposed to struck-by and caught-by hazards while operating a Cincinnati Brickford brand radial drill identified as Drill #4 without a point of operation guard.       e. This violation occurred on or about June 24, 2014, in the fabrication shop where employees were exposed to struck-by and caught-by hazards while operating a Carlton brand radial drill identified as Drill #5 without a point of operation guard.        Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all the point of operations on all machines are guarded.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    The employer does not develope procedures for the control of potentially hazardous energy.     a. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a Carlton brand radial drill, Drill #1, without having an energy control program that included effective energy control procedures.     b. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a Unknown brand radial drill, Drill #2, without having an energy control program that included effective energy control procedures.    c. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a Unknown brand radial drill, Drill #3, without having an energy control program that included effective energy control procedures.    d. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a Cincinnati brand radial drill, Drill #4, without having an energy control program that included effective energy control procedures.    e. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a  Carlton brand radial drill, Drill #5, without having an energy control program that included effective energy control procedures.    f. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a 64" Bullard Cutmaster vertical turret lathe, Cutmaster, without having an energy control program that included effective energy control procedures.    g. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a 155" Cincinnati vertical turret lathe, Big Cincy, without having an energy control program that included effective energy control procedures.    h. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a 110" Cincinnati vertical turret lathe, Baby Cincy, without having an energy control program that included effective energy control procedures.    i. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a 180" Betts vertical turret lathe , Betts, without having an energy control program that included effective energy control procedures.    j. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a 48" King vertical turret lathe , King, without having an energy control program that included effective energy control procedures.    k. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a 194" Niles vertical turret lathe , Niles, without having an energy control program that included effective energy control procedures.    l. This violation occurred on or about June 24, 2014, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing servicing and/or maintenance including setting up on a Warner and Swasey 4A horizontal turret lathe without having an energy control program that included effective energy control procedures.    Cab Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 315545939, citation number 1 item number 1 and was affirmed as a final order on July 15, 2011, with a final abatement date of August 31, 2011, with respect to a workplace located at 2306 Rayburn Drive, Nacogdoches, TX 75964.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that procedures are developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in activities covered by this section.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer does not guard all the rotating parts of machines to protect the operator and other employees in the machine area.         a. This violation occurred on or about June 24, 2014, in the Fabrication Shop where employees were exposed to struck-by and caught-by hazards while walking near and working with a 64" Cut Master brand vertical turret lathe identified as Cut Master without a guard to protect the operator from the rotating chuck or flying chips and sparks.          b. This violation occurred on or about June 24, 2014, in the Fabrication Shop where employees were exposed to struck-by and caught-by hazards while walking near and working with a 194" Niles brand vertical turret lathe identified as Niles without a guard to protect the operator from the rotating chuck or flying chips and sparks.       c. This violation occurred on or about June 24, 2014, in the Fabrication Shop where employees were exposed to struck-by and caught-by hazards while walking near and working with a 155" Cincinnati brand vertical turret lathe identified Big Cincy without a guard to protect the operator from the rotating chuck or flying chips and sparks.       d. This violation occurred on or about June 24d, 2014, in the Fabrication Shop where employees were exposed to struck-by and caught-by hazards while walking near and working with a 110" Cincinnati brand vertical turret lathe identified as Baby Cincy without a guard to protect the operator from the rotating chuck or flying chips and sparks.       e. Cab Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 315545939, citation number 1 item number 2 and was affirmed as a final order on July 15, 2011, with a final abatement date of July 15, 2011, with respect to a workplace located at 2306 Rayburn Drive, Nacogdoches, TX 75964.         Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all machines that have exposed rotating parts are guarded.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that the person authorized to perform the necessary sorbent bed and filter changes filled out a tag indicating by the signature and date that maintenance changes were performed:  The employer does not ensure that sorbent bed and filter changes are documented on a tag to indicate when the changes were performed. This violation occurred on or about June 24, 2014, in the sandblasting area, where an employee performs sandblasting operations using a supply air sorbent bed filter for breathing air without a tag identifying when filter changes had occurred.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure sorbent bed and filter changes are made by authorized personnel when necessary and the changes are documented when performed.
