29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) Woodsage Holdings, LLC located at 7400 Airport Hwy. Holland, OH 43528; On or about June 25, 2014, the employer did not utilize written energy control procedures on the Haven #6 pipe machine. Employees are required to clear jams between the unloader and the haven head while the machine is not locked out and are exposed to crush injuries from the incoming pipes and amputation injuries from the feed rollers. a.) Woodsage Holdings, LLC located at 7400 Airport Hwy. Holland, OH 43528; On or about June 25, 2014, the employer did not utilize written energy control procedures on the Haven #4 pipe machine end finisher. Employees are required to clean, clear pipe elevator jams, and change the insert while the equipment is not locked out and are exposed to caught by and amputation hazards.

Woodsage Holdings, LLC located at 7400 Airport Hwy. Holland, OH 43528; On or about June 25, 2014, the employer did not assure employees were protected from accidental start-up of the Haven #6 pipe machine. Employees are required to perform unjamming activities and do not lock out the electrical or pneumatic energy of the valve gates, exposing employees to crush or amputation hazards. b.) Woodsage Holdings, LLC located at 7400 Airport Hwy. Holland, OH 43528; On or about June 25, 2014, the employer did not assure employees were protected from accidental start-up of the Haven #4 pipe machine end finisher. Employees are required to performing cleaning and servicing activities and do not lock out the electrical energy, exposing employees to caught by and amputation hazards.

Holland, OH; On or about June 25, 2014, the employer did not assure employees were protected from pinch points and crush hazards between the unloader and the haven head on Haven #6. On May 22, 2014, an employee's hand was crushed between two pipes after he reached over the guard to clear a jam.
