29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used.        On or about June 24, 2014, the employer did not ensure the scaffold had a safe means to access and exit the working platforms exposing the employees to a fall hazard of more than 10 feet to the ground below.

On or about June 24, 2014, the employer did not ensure a mid-rail was installed along all open sides according to the requirements  in Appendix A exposing the employees to a fall hazard of more than 10 feet to the ground below.     The Midwest Drywall Company, Inc. dba MDC Drywall, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 314528209, citation number 01, item number 002 and was affirmed as a final order on 10/12/2010, with respect to a workplace located at 2700 South Shackleford Rd. Little Rock, AR 72205.
