29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Facility Wide - The employer failed to establish a program (lockout/tag out) to control hazardous energy that included energy control procedures for employees to follow, employee training, and periodic inspections to protect employees performing set up and maintenance/servicing on equipment such as the band saws, manual lathes, CNC lathes/turning centers, and similar equipment from injury caused by unexpected start up and energy releases. In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. The machines were not provided with machine guarding protection such as interlock devices allowing the sliding barrier doors to be opened and the machines to continue operating.
