29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219:   The employer does not guard all belts, pulleys, gears, shafts, and moving parts.      a. This violation occurred on or about June 24, 2014, on Line #1 where employees were exposed to struck-by and caught-by hazards while operating a conveyor table attached to Hydormat #23 without guarding to cover chains and sprocket wheels on the conveyor.     b. This violation occurred on or about June 24, 2014, in the Green Hydormat room where employees were exposed to struck-by and caught-by hazards while working near the Green Hydormat without guarding to cover an exposed shaft end on the machine.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all machines are equipped with power transmission safeguards.

29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of woodworking machines:   The employer does not guard the point of operation wood working machines.     a. This violation occurred on or about June 24, 2014, in the value added area of the facility where employees were exposed to a struck-by hazard when operating a miter saw without a guard to cover an exposed portion of the point of operation.     b. This violation occurred on or about June 24, 2014, in the weather stripping area where employees were exposed to a struck-by hazard when operating a weather strip trimmer without a point of operation guard.        c. This violation occurred on or about June 24, 2014, next to the paint coating area where employees were exposed to a struck-by hazard while working near a Jenkins Tenoner without guard to cover the backside of the point of operation.        d. This violation occurred on or about June 24, 2014, in the Green Hydromat room where employees were exposed to a struck-by hazard when operating the Green Hydromat without a point of operation guard.        e. This violation occurred on or about June 24, 2014, in the maintenance area where an employee was exposed to a struck-by hazard when operating a Jet brand drill press without a point of operation guard.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all the point of operations on all machines are guarded.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  The employer does not ensure that employees required to operate forklifts are reevaluated to ensure they can operate them safely. This violation occurred on or about June 24, 2014, and at times prior thereto, when employees in the facility were required to operate forklifts without being evaluated at least every three years, to ensure they could do so safely.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees required to operate forklifts are evaluated on the performance of its operation at least every three years.
