The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees are exposed to crushing hazards. The employer is failing to protect employees from struck-by and crushing hazards while performing waste collection activities. An employee was crushed between the vehicle and a utility pole when the vehicle was pulling into a tight alleyway. The driver was avoiding pedestrians and the employee distracted by another company vehicle in the area. Among other methods, feasible and acceptable methods of abatement are: 1) Ensure that all employees are re-trained on hazards related to this type of operation.
