29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not of the three-wire type:  a.   On or about June 25, 2014, at the temporary power service post, the extension cord used to power the microwave oven did not have a grounding prong, exposing employees to a 115-Volt, 8-Amp shock hazard from the ungrounded microwave oven.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:  a.   On or about June 25, 2014, at the top of the stairs in the left duplex unit, employees who accessed the second-floor were exposed to and 11 feet fall hazard in that a guardrail system was not installed on the stairs.  b.   On or about June 25, 2014, in the roof trusses of the second-story of the left duplex unit, an employee installing bracing was exposed to a 9 feet fall hazard to the floor in that he was not protected by a fall protection system.
