29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) For employees working at 3576 Bayou Louise Lane in Sarasota, as observed on or about June 25, 2014. Employees were installing solar panel connectors on a roof with a 5:12 pitch roof. The eave to ground height was approximately 16 feet 9 inches at the point of entry and approximately 26 feet at the eave where the anchor points were installed. The employees were not using fall protection or an alternative means of protection while installing the anchor points.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. a) For employees working at 3576 Bayou Louise Lane in Sarasota, as observed on or about June 25, 2014. Employees were installing solar panel connectors on a roof with a 5:12 pitch roof. The employees were using anchor points designed for single person protection and were not installed in accordance with the manufacturers instructions. Each anchor point was supporting two employees and were only rated for one employee each. b) For employees working at 3576 Bayou Louise Lane in Sarasota, as observed on or about June 25, 2014. The anchor points installed on the fascia for employee protection were not installed in accordance with manufacturers instructions. Manufacturer states in installation guide to never install on the fascia as the anchor installation is not capable of supporting at least 5000 pounds per employee attached.
