Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:           On or about June 25, 2014 at the above addressed jobsite, employees were using a pneumatic nail gun and were not wearing safety glasses, thereby exposing the employees to the hazards associated with eye injuries.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   On or about June 25, 2014 at the above addressed jobsite, employees were performing roofing operations approximately 19.7 feet above the ground and were not using fall protection, thereby exposing the employee to fall hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  On or about June 25, 2014 at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
