The violation occurred on or about June 18, 2014 and at times prior thereto in the MeadWestvaco chloride dioxide facility where employees were exposed to inhalation, fire, and explosion hazards when the employer failed to document the design codes and standards used in testing interlocks.      b.     The violation occurred on or about June 18, 2014 and at times prior thereto in the MeadWestvaco chloride dioxide facility where employees were exposed to inhalation, fire, and explosion hazards when the employer failed to document the design codes and standards used in the design of the ventilation system for the chloride dioxide plant building. This violation occurred on or about June 18, 2014 and at times prior thereto in the chlorine dioxide area where employees were exposed to fire and explosion hazards where the employer did not develop an implement operating procedures that address consequences of deviations from operating limits in the covered process.    Pursuant to 29 C.F.R. 1903.19, within (10) calendar days of the abatement date the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to develop and implement written operating procedures that address consequences of deviations from operating limits in the covered process.

29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel. These safe work practices shall apply to employees and contractor employees.    The employer does not ensure that safe work practices are developed and implemented for the control over entrance into the chloride dioxide process area by maintenance, contractor and other support personnel.  This violation occurred on or about June 18, 2014 and times prior thereto, in the chloride dioxide process area where this condition exposed employees to fire and explosion hazards when the employer failed to ensure that safe work practices were developed and implemented for the control over entrance into the chloride dioxide process area by maintenance, contractor and other support personnel.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure that safe work practices are developed and implemented for the control over entrance into the chloride dioxide process area by employees, maintenance, contractors, and other support personnel.
