29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    a.  On or about June 24, 2014, at the above addressed worksite, an employee was exposed to an electrical hazard while a fan (i.e. Hampton Bay, Model No. HV-18) that lacked a ground prong on its attachment plug was energized using a 2-wire extension cord.

On or about June 24, 2014, at the above addressed worksite, an employee was exposed to an electrical hazard while energizing a fan (i.e. Hampton Bay, Model No. HV-18) that lacked strain relief where its cable entered into the attachment plug.

On or about June 24, 2014, at the above addressed jobsite, employees were exposed to a struck by hazard while a powered industrial truck (i.e. Clark, Serial No. C365L-0097-9402FB) was operated with non-functioning back up alarm and front head lamp.
