On or about June 26, 2014, at the die making department of the above addressed worksite, an employee was exposed to a struck by hazard while using compressed air for cleaning purposes at a pressure above 100 psi.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   a.  On or about June 26, 2014, at the die making department of the above addressed worksite, an employee was exposed to a struck by hazard while working with a bench grinder (i.e. Baldor, Serial number #W7-91) that lacked tongue guards.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a.  On or about June 12, 2014, at the die cutting department of the above addressed worksite, an employee was exposed to an electrical hazard while working with a band saw (i.e. Ryobi, Serial Number BS104558949) that lacked a ground prong on its attachment plug.   b.  On or about June 26, 2014, at the gluer department of the above addressed worksite, an employee was exposed to an electrical hazard while working with tapping machine (i.e. Little David, Serial Number 380831537M) that lacked a ground prong on its attachment plug.
