29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    a) On the Slacker Tank, where the ladderway opening was not protected with a swing gate or offset to prevent employees from falling to the platform below.    b) On the Hydrate Silo, where the ladderway opening was not protected with a swing gate or offset to prevent employees from falling to the ground below.

29 CFR 1910.132(a): Protective equipment was not provided used whenever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) At the facility, where the proper personal protective equipment for employees' arm extremities was not provided and used to protect the employees from the caustic lime slurry.  The lime slurry has pH of 12.4.

29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:  a) In the pit area, where there were no caution signs posted to warn employees of the caustic conditions of the lime slurry pits.   29 CFR 1910.1200(f)(9): The employer did not ensure that labels or other forms of warning were legible, in English, and prominently displayed on the container, or readily available in the work area throughout each work shift:    a) Near the Washout Pit, where the Danger sign on the above ground storage tank was not legible and was covered with dried lime slurry.    b) Near Pit 2, where there was no displayed warning label identifying the lime slurry that was stored within the area.
