OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a debilitating arm injury when he drove forklift he was operating out of the trailer he was  loading, fell out of the trailer, and the drivers' arm was caught between the lift and the dock.   (a) Shipping dock 35 - A freight handler driving a Crown SC4520-35 forklift was injured while backing out of an out bound tractor trailer as the trailer started to pull away from the shipping dock. The forklift truck fell backwards off of the trailer four feet onto the pavement below, on or about 6-24-14.     Abatement Certification and Documentation Required.
