29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: On or about June 24, 2014, at the shipping area of Eagle Suspensions, the employer did not provide lock-out/tag-out equipment to employee who was required to perform lock-out/tag-out of energy sources while working on an overhead crane. The employee was exposed to hazards caused by unexpected start-up of machines or equipment including struck-by and caught-in.
