29 CFR 1926.451(g)(1)(ii):  Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:  a)  Two-point Suspension Scaffold at West Side of Building, 467 Valley Road, Maplewood, NJ.  Employee was not protected by a personal fall arrest system exposing the employee to 17 foot falls to the ground below.  Violation observed on or about 06/23/2014.

Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights.  a)  Two-point Suspension Scaffold at South Side of Building, 467 Valley Road, Maplewood, NJ.  The two vertical lifelines for this scaffold were not fastened to a safe point of anchorage as the lines were wrapped around piping systems or conduit which are not suitable anchorage points.   Violation observed on or about 06/23/2014.

29 CFR 1926.454(a):   The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a)  467 Valley Road, Maplewood, NJ and at the Corporate Offices at 14 New Street, Morristown, NJ.  The employer had not provided training on suspension scaffold hazard recognition and prevention to workers operating the two-point suspension scaffolds.  Violation observed on or about 06/23/2014.
